Health Point Occupational Rehabilitation

Program Overview

HealthPoint Physical Therapy of San Diego/Mission Valley is proud to provide Occupational Rehabilitation. It is based on a systems approach to occupational rehabilitation and will provide modified functional capacity evaluations in concert with current evidence based intervention including, but not limited to: cardiovascular health, body mechanics, postural education, proper lifting technique and work simulation. An integral aspect of the Occupational Rehabilitation at HealthPoint is returning the client to a physically employable status in an appropriate time frame, and teaching the client safe work habits so that he/she will return to work an educated, safe, and productive worker.

Occupational Rehabilitation

The program facilitates the transition from patient to worker through whole body conditioning, education and work simulation as the client builds a greater work tolerance.

Work Conditioning

The program is designed to increase the worker’s physical health and function through strength, endurance, and flexibility exercise. The severely deconditioned client is readied for return to work or participation in a transitional duty program.

Schedule

  • Minimum of two (2) hours per day five (5) days per week or as per MD script
  • Maximum of four (4) hours per day five (5) days per week or as per MD script
  • Monday through Friday for four (4) to six (6) weeks

Work Transition

  • Clients with a pre-existing work opportunity
  • Goal: Return client to work in an organized manner with a weekly schedule set by PT, MD, case manager, employer and adjuster
  • Decrease time at Occupational Rehabilitation/conditioning and increase work time
  • Clients without a pre-existing work opportunity
  • Goal: Return the client to, or as close to their pre-injury status as possible

Referrals

The standard Health Point Physical Therapy form may be used for all new referrals. For your convenience, phone referrals will also be accepted.

Orientation

The client will undergo a modified intake FCE and will be personally oriented to the goals and processes of the program.

Absenteeism Policy

Clients are allowed two (2) unexcused absences before being discharged from the program. Client must call personally if he/she is going to be late or absent.

Client Status

Bi-weekly progress notes will be mailed, faxed or e-mailed to the physician, case manager or other pertinent parties. Weekly progress notes are available if requested.

Discharge

Detailed discharge summaries including functional capacity evaluation information will be mailed, faxed or e-mailed to the physician, case manager and or other pertinent parties within twenty-four (24) hours of the completion of Occupational Rehabilitation.
